We declare that the above information is true, correct and complete and is given to induce the Al-Amin Brothers Transportation, LLC to extend credit. We authorize the Al-Amin Brothers Transportation, LLC to make such credit investigation as Al-Amin Brothers Transportation, LLC sees fit, including contacting the above trade references and banks and obtaining credit reports. We authorize all trade references, banks and credit reporting agencies to disclose to the Al-Amin Brothers Transportation, LLC any and all information concerning the financial and credit history of my company and myself:

In addition, if credit is granted, we agree to pay Al-Amin Brothers Transportation, LLC their invoices within the terms net 15 days from receipt of invoice. If payment is 10-15 days past due, Al-Amin Brothers Transportation, LLC has the right to delay any and all orders on hand until account is current. If for any reason our account is turned over to collections, we will be liable for all fees incurred including attorneys fees and court costs.
